Accessing our support

There are many ways in which Cosgrove can provide you with support.

There are two key routes to engaging us to provide support:

1. Through a referral

Cosgrove work in partnership with Local Authority Health and Social Care Partnerships to ensure the most appropriate match of individuals to our support services. We work alongside people and their representatives to design a person-centred and outcome focused plan to best suit individual needs.

Individuals, parents and carers can contact us for information and we welcome your involvement in the referral process.  Either give us a call, email us or fill out the call back form at the top of this page.

2. Self-Directed Support

Cosgrove Self-Directed Support is an impartial service which give you all the information you need to choose what is best for you and your family.

When we meet with you, we will help you to make an informed choice about what support you believe will help make life easier for you.  We will also discuss your individual care budget and how you would like your support provided.  We can help you to agree on a support plan to guide what money, services and equipment you need.

If you choose Cosgrove to provide your support, we will agree when it starts and how you might wish to be involved in recruiting the staff you need.  We will review your support on an ongoing basis and respond to any changes as quickly as possible.
